New Patients

We welcome new patients who are living within our practice area.  The practice has an inner and outer boundary.  Our inner boundary is the area bounded by Kings Hedges Road, Histon Road, Gilbert Road and Milton Road.  The outer boundary includes all properties north of Kings Hedges Road and west of Milton Road up to the A14.

If you live within our practice boundary, please download our registration forms & return them in person to our Reception team.

Please note you will not automatically be registered on the day therefore please do not expect to be seen immediately.

We require photo identification (e.g. passport or driving licence) for full registration & you will therefore be required to apply in person at our reception desk.

And either  Adult Registration Form  or Under 5’s – Registration Form
